IMPACT-ALL aims to improve treatments for acute lymphoblastic leukemia (ALL) in children and adults through multi-omic analyses, innovative targeting of high-risk subgroups, and novel immunotherapy.

Aim 1 focuses on identifying new genetic, immunophenotypic, and metabolic biomarkers by analyzing patients with ALL, focusing in resistant/relapsed cases. Techniques include transcriptomics, immunophenotyping, DNA copy number, and single-cell profiling via CyTOF. Minimal residual disease (MRD) monitoring will help identify resistance pathways, while a humanized bone marrow-like model will further characterize resistance mechanisms. Machine learning will be used to integrate clinical data for personalized treatment.

Aim 2 aims at precision targeting for high-risk resistant/relapsed ALL. High-risk cases will be identified and targeted using engineered cell models, xenografts, and mouse models, with a focus on overcoming drug resistance via large-scale drug screening.

Aim 3 enhances immunotherapy by developing next-generation CAR-T cells targeting CD22, BAFFR and CD79b, aiming to improve persistence and safety. Preclinical studies will utilize the humanized bone marrow model to evaluate treatments, while spectral flow cytometry will monitor immune reconstitution post-treatment.

IMPACT-ALL aims to providing the health system with a framework to expand therapeutic options for r/r ALL patients, by innovative strategies to be transferred into better outcome and quality of life.
