Brain metastases (BrM) from breast cancer (BC) remain a major clinical challenge due to poor prognosis and limited treatment options. There is an urgent need for new markers to predict BrM occurrence, identify high-risk patients early, and effectively prevent or treat BrM. BrM formation involves BC cells overcoming several critical hurdles, such as crossing the blood-brain barrier, surviving in the brain’s unique microenvironment, and adapting to proliferate within this specialized niche.

This project utilizes a comprehensive multi-omic approach—including radiomics, transcriptomics, genomics, epigenomics, and metagenomics— across different clinical sample cohorts, including both matched and non-matched BrM, metastasis from other sites and primary tumors. The goal is to discover novel integrative biomarkers that specifically predict BC metastasis to the brain and to identify key drivers, pathways and bacteria involved in BrM formation. Innovative in vitro and in vivo models of BrM will be developed to validate the functional roles of these drivers, determining their contribution to various stages of metastasis and guiding the development of targeted therapies to disrupt these processes.

Ultimately, this research will identify clinically impactful biomarkers, offer a deeper integrative understanding of BrM, and open new avenues for developing effective therapeutic strategies to combat BrM.
